The video below was produced by STAMP as a training tool to help our trainees understand and appreciate what happens when a pressure release occurs due to the failure of a threaded plug, an ever-present hazard in our industry.
Thread failure in plugs can occur for a number of reasons such as damaged threads on the male or female side, mis-matching threads or over-use of PTFE thread tape. When the failure does occur the effects can be catastrophic, the plug becomes a high velocity missile accompanied by a pressure blast capable of causing severe injury and plant damage.
