Uncontrolled release of compressed springs from actuators can occur due to excessive corrosion, material defects or from using an unsafe disassembly procedure. This link is to a safety alert describing an uncontrolled release of spring energy from an actuator.

When a compressed spring is released in an uncontrolled manner there is the potential for serious harm for anyone in the line of the force and for substantial damage to be caused to surrounding plant, the video above shows how the release occurs and how violent it can be. The spring used in this simulation was relatively small and the energy release therefore is small in comparison to the energy which would be released by larger actuator springs.
